To the Editor The Frequency of Screening and Spontaneous Breathing Technique (FAST) trial provides important evidence to guide successful extubation for critically ill patients. However, we highlight a deviation from the intention-to-treat analysis principle, its potential effect on reported outcomes, and its ramifications for clinical trials of critically ill populations.
