In Reply We thank Dr Andrea and colleagues for their comments on the postrandomization exclusions and withdrawals in our trial. Figure 1 documented 40 of 837 (4.8%) patients included in our modified intention-to-treat analysis; of which, 28 (3.3%) were excluded postrandomization and 12 (1.4%) were withdrawn because we lacked permission to include their data. The frequency of postrandomization exclusions and withdrawals was similar across treatment groups.
