Unilateral Do-Not-Resuscitate Orders

To The Editor In their Viewpoint, Dr Piscitello and colleagues argue that explicit documentation of unilateral do-not-resuscitate (UDNR) orders is an ethical obligation. They propose a conceptual framework recognizing 2 distinct forms of UDNR decision: the conflict UDNR decision, made “despite active opposition expressed by the patient or surrogate,” and the assent UDNR decision, whereby a DNR decision is communicated to patients or surrogates who do not express, but may have, disagreement.
